Diet-related non-communicable diseases and associated factors is an extensive topic, what I suggest, you should be particular to one or two diseases then relate them with diet and other factors. Later you develop a questionnaire by yourself and then ask the colleagues for more questions related to your study diseases and finally compile all questions into one.
