Try and check with hospitals for records of malnutrition related diseases. Then if possible also do household surveys. 

If you are talking about prevalence you may have to do survey in which you will have to conduct anthropometric assessment of under five. Alternatively you can use MUAC tape for rapid assessment depending on what you want to use the data for.
